ONLINE REGISTRATION

To attend CISAP5 conference a registration fee is required.
For publishing a paper and including it into the final program the registration fee is due before April 15
The fee includes conference facilities; proceedings (hard copy + CD-Rom) ; welcome cocktail, 5 coffee breaks;3 lunches, 1 dinner

The fee will not include accommodation.

If you choose a vegetarian menu please inform the secretariat by sending a separate mail after the submission of this form, otherwise it will be assumed that you have chosen the non-vegetarian menu.

The deadline for early registration at a reduced fee refers to the payment date and not to the online registration submission date.

Cancellations before May 19th, 2012 will be charged by 20% costs. Beyond such date the registration fee will not be refunded. Only written cancellations will be accepted (letter or fax to the postal address of the CISAP5 Secretariat, not e-mail).

Substitutions will be accepted at any time, provided they are notified to the Secretariat in advance.
We reserve the right to cancel or alter the program.
